Role OverviewAs a Discharge Planner at Saint Clare's Health, you will be responsible for coordinating the activities of the Case Management Department and assisting with discharge planning. You will work under the direction of a Case Manager/Social Worker and assist with case management referrals and authorizations.
What You Will Do
Your main responsibilities will include interviewing patients/families, gathering data, and developing safe patient-focused discharge plans. You will also assist with Medicaid state submission forms and maintain facility procedures.
Why It Might Be a Fit
This role requires knowledge of Discharge Planning/Utilization Management/Case Management terminology and functions. You should have basic to intermediate computer skills and a current BCLS (AHA) certificate. Experience in the hospital setting is preferred.
