Role Overview

As a Discharge Planner, you will be responsible for assessing psychosocial needs, providing counseling, and facilitating comprehensive care plans to ensure successful community re-entry for our residents. You will work effectively with interdisciplinary teams, residents, families, and designated representatives.

What You Will Do

Assess psychosocial needs, provide individual, group, and family counseling, facilitate comprehensive care plans, and explore discharge planning/community options.

Requirements

  • Bachelor of Social Work (BSW) degree or Bachelor degree in a related field
  • One year experience in social work or case management
  • Professional attire and clinical communication skills
  • Experience with office technologies, computers, word processing, and software programs
  • Good organizational skills, pleasant and professional telephone persona, and ability to communicate well with department staff
  • Ability to read, write, speak, and understand the English language
