Job Description
The Department of Mental Health is seeking a Child Youth and Family Case Manager to provide case management services for eligible youth under age 22. The successful candidate will have knowledge and desire to work with youth with mental health needs and their families to promote recovery and connection to community-based services.
Requirements
Provide case management services for eligible youth under age 22, with majority of caseload below age 18.
Engage families/guardians in the process to align with CYF Family Collaboration Principles.
Work with a team of internal and external providers and family to ensure integrated planning and service provision.
