Role OverviewThe Hospital Social Worker at MultiCare is responsible for cross continuum care coordination, psychosocial assessments, and psychosocial interventions. This role identifies opportunities to continually improve patient care, services and population health. The position is based at MultiCare Capital Medical Center, a 107-bed community hospital in Olympia, Washington.
What You Will Do
You will perform cross continuum care activities across various sites of care, provide customer service to patients and families regarding discharge milestones, and model consistent professional communications. You will delegate responsibilities appropriately while remaining accountable for outcomes, and ensure medically appropriate transitions of care.

Requirements
- Master's degree in Social Work
- Licensed in the state of Washington
- Licensed Independent Clinical Social Worker (LICSW); OR Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain their LICSW within four (4) years
