Role OverviewThe Enhanced Care Management Coordinator supports the delivery of enhanced care management services for high-need Medi-Cal members served by TMWIHC. Reporting to the Tribal Registered Nurse, this position coordinates comprehensive care plans, facilitates interdisciplinary care team collaboration, and connects members with community resources to address social determinants of health and improve health outcomes.
What You Will Do
Serve as the primary care coordinator for high-need Medi-Cal members enrolled in the Enhanced Care Management program, conduct comprehensive needs assessments, develop individualized care plans, and connect members with community resources.

Requirements
- Bachelor's degree in social work, nursing, public health, psychology, or a related field
- one year of care coordination or case management experience
- Valid California Driver's License and personal automobile insurance meeting TMWIHC minimum coverage requirements
- CCM (Certified Case Manager), LSW, or equivalent certification preferred
