Job Description
Pacific Health Group seeks a Lead Care Coordinator to manage a caseload of approximately 60-70 members, conducting in-person visits and coordinating care across medical, behavioral health, and community services. The successful candidate will be able to navigate complex healthcare systems, build trust with members, and advocate for timely access to care and resources.

Requirements

  • 3-5 years in case management, social services, or healthcare
  • Experience with Medi-Cal, CalAIM, or Enhanced Care Management
  • Working experience of healthcare systems and community resources
  • Valid California Driver’s License and active auto insurance
  • Reliable personal vehicle for daily work use
  • Successful completion of background check
  • Ability to travel up to 60-70% within the county
  • Ability to effectively communicate both written and verbally
  • Proficiency with documentation systems and technology
