Job Description
Family Nurse Practitioner/Physician Assistant will provide professional medical services to patients, manage population health, and lead the patient-centered medical home care team.

Requirements

  • Graduation from an accredited Nurse Practitioner advanced degree program
  • Two (2) years’ experience in an ambulatory care setting, preferred
  • Certified Nurse Practitioner license from the State of Arizona
  • Current DEA Certificate renewed as required by law
  • Current CPR certification renewed as required by law
  • Current Fingerprint Clearance Card
