Job Description
A Family Resources Coordinator is sought to provide critical support and service coordination for children with identified delays through the Early Support for Infants and Toddlers program.
Requirements
Assists families in identifying their children's strengths and needs
Completes monthly check-in calls with families
Maintains complete documentation of the child's case records
Works with referring agencies in the development and implementation of individual treatment plans
Provides care and program planning, coordination, and administration to support Medicaid programs
Maintains relationships with social service agencies and family service providers in the community
