Role OverviewThe Nurse Navigator / Field Case Manager Hybrid is responsible for the day-to-day management, coordination, and supervision of the related clinical activities within their assigned service area.
What You Will Do
Expected to routinely complete face to face patient visits and staff visits in the home setting for all patients on caseload, build face to face rapport with patients and patient families, and maintain communication with Regional Clinical Director, supervisor, case managers, and other staff.

Requirements
- Bachelor of Science in Nursing preferred
- Current RN licensure in the state of Nevada
- A valid driver's license and be in good standing
