Job Description
Field Sales Representative for Monogram Health, engaging face-to-face with patients to educate them on Monogram Health services and connect them to life-changing care, transforming care for high-risk patients with multiple chronic conditions.

Requirements

  • Conduct daily in-person outreach to high-risk, hard-to-reach patients by visiting their homes unannounced
  • Proactively schedules and completes at least 15 home visits daily
  • Meet or exceed sales targets and KPIs
