Role OverviewProvide Community Hab services as indicated on the Individuals Service Plan (ISP) and Community Hab plan. Implement the goals outlined in the Community Hab Staff Action plan. Maintain records of the individuals being served in the Community Hab program and provide them to the Home & Community Services Supervisor weekly.
What You Will Do
Implement goals, maintain records, observe individuals for unusual changes, report to supervisor, attend training, and demonstrate flexibility, accuracy, and confidentiality.

Requires experience working with developmentally disabled, valid New York State Driver's License, and ability to maintain confidentiality.
Requirements
