Role OverviewThe CDS Support Specialist is responsible for supporting the CDS program with Addus Home Care, allowing individuals to age in place and receive care at home.
What You Will Do
Monitor and maintain monthly supervisory call lists, schedule intakes and annual home visits, manage time to ensure timely completion of tasks, and maintain accurate documentation.

Requirements
- High School diploma with some college preferred
- Familiarity with home and community-based services and regulations
- Proficient in Microsoft Office programs (Word, Excel, PowerPoint)
- Valid driver's license and proof of insurance
