Role OverviewThe Licensed Practical Nurse is responsible for providing nursing care services and case management to clients and families by implementing nursing interventions set forth by the plan of care.

Provides direct patient care, implements plan of care, provides accurate and timely documentation, and assesses and provides patient and family/caregiver education.

Requirements
- Degree in nursing field as a licensed practical nurse
- 12 months facility/agency experience in nursing as an LPN preferred
- Valid driver's license, automobile insurance, and qualified to drive under the organization's motor vehicle check
- Good verbal and written communication, comprehension, computer, and interpersonal skills
- Ability to speak, read, write, and understand English
- Current LPN license in good standing in the state(s) in which practicing
