Role OverviewFunctional Eligibility Screener will perform initial, periodic, and change of condition functional screens on Family Care, PACE, and Partnership Program members. They will also provide training to certified staff and assist new Care Management staff in obtaining LTCFS certification.
What You Will Do
Perform functional screens, provide training, track screen completion, and participate in team meetings. Regular local travel is required.

Requirements
- Bachelor's Degree in Social Work, Psychology, Human Services, or related field
- Current Long-term Care Functional Screener certification
- 18 months of experience in a social service capacity or a Master's degree in Social Work, Psychology, or Human Services and one year experience with older adults or persons with physical and or intellectual disabilities in a social service capacity
