Role OverviewThe Case Manager provides intensive case management services to gang-involved youth and families impacted by gang violence. The Case Manager is the primary point of contact to coordinate services and care for assigned clients.
What You Will Do
Conducts comprehensive screenings and assessments with individuals and families to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate, to develop a case plan. Serves and engages gang-involved youth or young adults, ages of 14 to 25 years in a GRYD Zone.
Why It Might Be a Fit
Ability to work and deal with difficult, uncooperative, and potentially aggressive clients as VOALA serves the neediest of the needy in our programs, which may include clients with homelessness, mental illness, substance abuse, recently released from incarceration, and other challenges.
Requirements
- Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
- Ability to perform under circumstances of possible emotional stress and conflicts dealing with difficult, uncooperative, and potentially aggressive clients
- 2 years' experience working with At-risk families/individuals or gang population
