Job Description

Requirements

  • DDS/DMD degree from an approved dental school
  • Current NYS licensure
  • Documentation of current BLS certification
  • DEA Controlled Substances Registration Certificate
  • Familiarity with New York Stated Dental Medicaid/ADAP guidelines
  • Certification of completion of oral cancer detection mandate course
