Habilitation Coordinator I

Requirements

• Implement individuals' programs through hands-on / direct service for a minimum of 25 hours of a forty (40) hour work schedule.
• Evaluate the effectiveness of programs through various data collection systems in order to monitor progress and ensure maximum growth.
• Document results of services rendered in accordance with the standardized agency policies and procedures to ensure effective communication on individual status.
• Follow through on requests and assignments initiated by supervisors to assist in the accomplishment of department goals.
• Direct, coordinate, and oversee the implementation of program services according to established guidelines to ensure the provision of Active Treatment.
• Clinical responsibilities will require assessing individuals in accordance with Department policy in order to determine appropriate educational training programs.
• Develop individual centered objectives and interventions in conjunction with other team members through participation in the Individual Service Plan (ISP) process and related clinical meetings to provide quality services to individuals.
• Monitor program staff in order to meet established individual objectives and assist in providing quality of life. Teach program staff general and specific approaches and plans to ensure quality program services.
