Job Description
This HCBA Patient Care Navigator position reports to the HCBA RN Case Manager Supervisor and independently performs assigned duties, interacting with participants, vendors, and support staff. The role focuses on coordinating care, facilitating appointments, and providing administrative support for a comprehensive care program. Emphasis is placed on excellent customer service and adherence to HCBA program guidelines.

Requirements

  • • Associate degree or higher, and 3-5 years of progressively responsible experience related to health plan operations.
  • • Demonstrated excellent customer service, written and oral communication skills.
  • • Basic knowledge of PC's and related software.
  • • Ability to perform duties in a heavy workload environment.
