Role OverviewThe Health Access Aging Services Coordinator plays a key role in supporting older adults living with HIV by facilitating health insurance access and related healthcare services. This position maintains an active caseload of 100+ clients over the age of 50 and provides one-on-one assistance with Medicare, Medicaid, private insurance enrollment, and Social Security Disability Insurance (SSDI) navigation.
What You Will Do
Educate clients and the public on SDAP and relevant updates to healthcare policy and access, conduct enrollment appointments, provide quarterly outreach and ongoing assistance to clients aging into Medicare, develop individualized Medicare and SSDI transition plans, and troubleshoot medication access and insurance issues.

Requirements
- Knowledge of HIV/AIDS, health insurance systems, and aging-related issues
- Experience assisting with healthcare navigation and benefit programs, including Medicare and Medicaid
- Bachelor's Degree in Social Work, Human Services, Public Health, Gerontology, and/or a related field or equivalent experience
- Excellent organizational and time management skills
- Demonstrated interpersonal and communication skills, both verbal and written
- Ability to work independently and as part of a team
- Commitment to culturally responsive and trauma-informed service delivery
- Strong attention to detail and ability to maintain confidential information
- Bilingual or multilingual candidates, especially Spanish-speaking, are strongly encouraged to apply
