Job Description
Role Overview

The Health Aide will provide personal care, light housekeeping, and assistance with ADLs (Activities of Daily Living) for participants in their homes and at the PACE center. The Health Aide will exercise independent judgment and report any changes in participant status to the IDT. The Health Aide will participate in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned.

What You Will Do

Primary responsibilities include assisting with ADLs, contributing to the development of a care plan, collaborating with the Interdisciplinary Team, carrying out non-skilled treatments, and maintaining a clean and safe working environment.

Why It Might Be a Fit

The ideal candidate will have one year of experience working with a frail and/or older adult population, preferably in a PACE program. Certification as a Home Health Aide or Nursing Assistant is desired, and reliable transportation is required.

Requirements

  • Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program
  • Certification as a Home Health Aide or Nursing Assistant
  • One year of experience working with a frail and/or older adult population
  • Reliable transportation
