Job Description
The State of Connecticut, Department of Public Health (DPH) is seeking a qualified Dietitian/Nutritionist, Clinical Social Worker, Occupational Therapist, Physical Therapist, or Speech Language Pathologist for a Health Facility Surveyor position in the Facility Licensing and Investigations Section (FLIS). The position will involve investigating complaints and unusual occurrences, conducting in-person visits to health care facilities, and evaluating quality of care, quality of life, and resident rights.
Requirements
Four (4) years of experience working as a registered Dietitian/Nutritionist in a position involving administration, consultation, education or supervision.
Four (4) years of experience as a licensed Clinical Social Worker, Occupational Therapist, Physical Therapist or Speech Language Pathologist in a position involving administration, consultation, education or supervision.
Registration as a Registered Dietitian (RD) through the Commission on Dietetic Registration AND certification as a Dietitian/Nutritionist issued by the Connecticut State Department of Public Health in accordance with Connecticut General Statutes Section 20-206n and 20-206r.
Licensure as a Clinical Social Worker in the State of Connecticut pursuant to Sec. 20-195 (m) through (r) of the Connecticut General Statutes.
Licensure as an Occupational Therapist issued by the Connecticut State Department of Public Health in accordance with the applicable Connecticut General Statutes.
Licensure as a Physical Therapist issued by the Connecticut State Department of Public Health in accordance with the applicable Connecticut General Statutes.
Licensure as a Speech and Language Pathologist issued by the Connecticut Department of Public Health in accordance with the applicable Connecticut General Statutes.
