Job Description
The State of Connecticut, Department of Public Health (DPH) is seeking a qualified Dietitian/Nutritionist, Clinical Social Worker, Occupational Therapist, Physical Therapist, or Speech Language Pathologist for a Health Facility Surveyor position in the Facility Licensing and Investigations Section (FLIS). The position will involve investigating complaints and unusual occurrences, conducting in-person visits to health care facilities, and evaluating quality of care, quality of life, and resident rights.

Requirements

  • Four (4) years of experience working as a registered Dietitian/Nutritionist in a position involving administration, consultation, education or supervision.
  • Four (4) years of experience as a licensed Clinical Social Worker, Occupational Therapist, Physical Therapist or Speech Language Pathologist in a position involving administration, consultation, education or supervision.
  • Registration as a Registered Dietitian (RD) through the Commission on Dietetic Registration AND certification as a Dietitian/Nutritionist issued by the Connecticut State Department of Public Health in accordance with Connecticut General Statutes Section 20-206n and 20-206r.
  • Licensure as a Clinical Social Worker in the State of Connecticut pursuant to Sec. 20-195 (m) through (r) of the Connecticut General Statutes.
  • Licensure as an Occupational Therapist issued by the Connecticut State Department of Public Health in accordance with the applicable Connecticut General Statutes.
  • Licensure as a Physical Therapist issued by the Connecticut State Department of Public Health in accordance with the applicable Connecticut General Statutes.
  • Licensure as a Speech and Language Pathologist issued by the Connecticut Department of Public Health in accordance with the applicable Connecticut General Statutes.
