Job Description
As a Health/Human Service Case Manager II, you will determine eligibility for various benefits, provide case management and counseling, and make referrals for benefits. You will also work closely with SNAP vendors and share information about benefits with clients in a timely and accurate manner.

Requirements

  • Six months' experience in case management
  • Six months' experience in making recommendations for client service plans
  • Six months' experience in interpreting laws, rules, and regulations
  • Six months' experience in using automated information systems
  • Six months' experience in narrative report writing
