Role OverviewThe Hepatitis C Linkage to Care Specialist will work with the Director of Nutrition and Operations to provide education and outreach for patients that are HCV positive. The role involves facilitating enrollment in HCV Benefits and Entitlement programs, maintaining data and case records, and educating community agencies and internal staff members about the importance of Hepatitis C testing and treatment.
What You Will Do
The responsibilities of the Hepatitis C Linkage to Care Specialist include facilitating enrollment in HCV Benefits and Entitlement programs, maintaining data and case records, educating providers and other staff about HCV testing, benefits of diagnosis, and importance of linking to care, and working cooperatively with entire HCV case management, Medical, and Behavioral Health Provider teams.
