High Fidelity Wraparound Facilitator

Requirements

• Provide on-going clinical management for all High Fidelity Wraparound referrals/cases.
• Maintain WF credentialing as required by the state of Indiana and comply with the High Fidelity Wrap model.
• Coordinate with necessary Southwestern Behavioral staff and other agencies/organizations on behalf of coordinating care and services for clients and their families.
• Ensure compliance with all Southwestern policies and procedures including clinical billing and documentation.
• Collect and report pertinent data as required by the state.
• Demonstrate the ability to assess and respond to psychiatric emergent situations.
• Maintain documents and submit reports according to established time frames.
• Attend and participate in regular staff meetings.
• Facilitate child and family team meetings in accordance with High Fidelity Wrap model.
• Participate in ongoing skills training and development activities as assigned to enhance job performance.
• Provide consultation and education activities to community and professional groups as needed.
• Maintain contact with other service providers, community agencies, schools and civic organizations to enhance community awareness and public relations.
