Role OverviewThe Holocaust Survivor Case Worker will work with Holocaust survivors who require case management to age in place, providing holistic services through a trauma-informed patient-centered practice model. This is a hybrid position with three days on-site and two days remote.
What You Will Do
Provide trauma-informed case management services, including financial assistance, benefits applications, and referrals. Offer support in clients' homes and at the main office, assist in organizing socialization events, and collaborate with partner agencies.

Requirements
- Bachelor's degree (BA, BS, or BSW)
- Fluency in Russian
- 2+ years of experience in case management working with the geriatric population, preferably Holocaust survivors or other trauma-impacted populations
- Proficiency in Microsoft Office, case management databases (e.g., Salesforce), and electronic record-keeping
