Role OverviewJoin Ambercare/Addus HomeCare to help older adults and individuals with disabilities live safely and independently at home. As a Service Coordinator, you will schedule and supervise in-home care workers and clients in a geographic area, conducting home visits to monitor and report on quality of services.
What You Will Do
Create work schedules, manage client schedules, contact care providers and clients, monitor client conditions, and maintain confidentiality. You will also conduct home visits, evaluate Home Care Aides, and prepare evaluation reports.
