Role OverviewClassic Home Care is seeking a motivated, outgoing, and community-driven Home Health Community Liaison to join our growing team. In this role, you will serve as the primary link between our agency and referral sources, healthcare partners, and the communities we serve across Southeast Michigan.
What You Will Do
Key responsibilities include building and maintaining relationships with referral sources, educating the community about Classic Home Care's services, conducting regular visits to referral partners, and representing the agency at community events.

Requirements
- Prior experience in home health, healthcare sales, community outreach, or a related field required.
- Familiarity with the Southeast Michigan healthcare market is a plus.
- Strong interpersonal and communication skills with a natural ability to build trust and rapport.
- Self-motivated with the ability to work both independently and as part of a collaborative team.
- Excellent organizational skills and attention to detail.
- Valid Michigan driver's license and reliable transportation required.
- Proficiency with basic computer skills; experience with EMR or CRM systems a plus.
- Knowledge of Medicare home health guidelines and eligibility criteria preferred.
