The OT Case Manager plans and provides skilled occupational therapy services to clients in their home, managing a schedule during office hours and traveling to client homes for 1-3 hour visits.

Requirements

  • Currently licensed as an Occupational Therapist in Minnesota
  • Graduate of AOTA approved school of occupational therapy
  • 2+ years experience in healthcare or equivalent
  • Home care experience preferred
  • Certification with National Board of Occupational Therapy recommended
