Role OverviewRemedy Home Health Care is seeking a licensed Physical Therapist to join their growing Home Health team in Los Angeles County. The Home Health Physical Therapist will work with senior care populations, delivering skilled therapy services in home-based, assisted living, and community care settings.
What You Will Do
As a Home Health Physical Therapist, you will conduct in-home physical therapy evaluations and reassessments, develop and implement personalized rehabilitation care plans, and provide therapy in home health, assisted living, and community care settings.

Requirements
- Licensed Physical Therapist (PT) – California
- CPR Certification (AHA approved)
- Minimum 1 year of clinical experience (Home Health preferred)
- Experience in senior care, geriatrics, rehab, or community-based care preferred
- Strong clinical, communication, and patient education skills
- Valid driver’s license, reliable transportation, and auto insurance
