Job Description
The Physical Therapy (PT) Case Manager provides and oversees home health services for clients within the community. Collaborates with the client, their family, and the health care team to develop and implement goal-centered care plans taking into consideration quality measures and financial aspects of treatment.

Requirements

  • Licensed as a Physical Therapist (PT) in the State of Minnesota
  • Proof of negative TB testing
  • Access to a dependable automobile
  • Valid driver’s license and proof of current car insurance
  • U.S. Citizenship or evidence of valid Alien Work Permit
