Job Description
As a Home Health Physical Therapy Case Manager at Adara Home Health, you will provide home health services for clients in the community, collaborating with the client, their family, and healthcare team to develop goal-centered care plans. This is a full-time position, Monday to Friday, with the opportunity to manage your schedule during office hours. The position requires traveling to client homes to provide one-on-one care, which may take 1-3 hours per client.

Requirements

  • Currently licensed as a Physical Therapist (PT) in the State of Minnesota
  • Proof of negative TB testing
  • Access to a dependable automobile
  • Valid driver’s license and proof of current car insurance
  • U.S. Citizenship or evidence of valid Alien Work Permit
  • Disclosure of any conviction and criminal history records
  • Pass initial and ongoing background studies and screenings
