As a Physical Therapy Case Manager, you will provide and oversee home health services for clients within the community, collaborate with clients, their family, and the healthcare team to develop and implement goal-centered care plans, and provide skilled care to clients and clinical oversight to professional and unlicensed personnel.

Requirements

  • Current licensure as a Physical Therapist in the State of Minnesota
  • 2 years of experience in a healthcare facility or home care field
  • Ability to work independently with minimal supervision
