Job Description
Adara Home Health offers a fulfilling Home Care career with flexibility and work-life balance. The Physical Therapy (PT) Case Manager provides home health services to clients and collaborates with healthcare teams to develop care plans. This is a full-time position with an on-call rotation.

Requirements

  • Currently licensed as a Physical Therapist (PT) in the State of Minnesota
  • Proof of negative TB testing: Mantoux skin test, single TB blood test or chest x-ray
  • Access to a dependable automobile and valid driver's license
  • U.S. Citizenship or valid Alien Work Permit
  • Ability to pass initial and ongoing background studies and screenings
  • 2+ years' experience in a healthcare facility or home care field
  • Critical thinking skills, ability to follow standards/procedures, and make clinical judgments
  • Ability to work independently, adapt to change, and manage multiple assignments
  • Excellent written and oral communication skills, interpersonal communication, and teaching skills
  • Basic computer skills and willingness to learn new software programs
