Job Description
As a Home Health Physical Therapy Case Manager, you will provide and oversee home health services for clients in the community, collaborating with clients, family, and healthcare teams to develop and implement goal-centered care plans. You will also provide skilled care to clients and clinical oversight to professional and unlicensed personnel.

Requirements

  • Currently licensed as a Physical Therapist (PT) in the State of Minnesota
  • Proof of negative TB testing
  • Access to a dependable automobile
  • Valid driver's license and proof of current car insurance
  • U.S. Citizenship or evidence of valid Alien Work Permit
  • Disclose any conviction and criminal history records
