Job Description

As a Physical Therapy (PT) Case Manager at Adara Home Health, you will provide and oversee home health services for clients, collaborating with the client, their family, and healthcare team to develop and implement goal-centered care plans. Requirements

Current licensure as a Physical Therapist (PT) in Minnesota
2+ years' experience in healthcare or home care
TB testing (Mantoux skin test, single TB blood test, or chest x-ray)
Valid driver's license and car insurance
U.S. Citizenship or Alien Work Permit
Background studies and screenings
