Role OverviewHalcyon Home is hiring a Home Health Physical Therapist Assistant to support our growing geriatric home health population in San Marcos, TX and surrounding areas. As a Physical Therapist Assistant, you will provide physical therapy to our patients in both community and residential settings.
What You Will Do
Perform ongoing clinical assessment, develop and evaluate the plan of care, document clinical notes, and instruct patients and families in transfer techniques. Provide patient, caregiver, and family counseling, and provide services ordered by the physician.

Requirements
- Current PTA license
- Knowledge or experience utilizing Homecare Homebase EMR
- Experience in home health or related healthcare setting
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
