Job Description
As a Home Health RN Case Manager at Adara Home Health, you will provide direct care and oversee home health services for clients in the community. You will collaborate with clients, family, and healthcare team to develop and implement goal-centered care plans. This is a full-time position with an on-call rotation, requiring travel to client homes and occasional heavy lifting.

Requirements

  • Currently licensed as a Registered Nurse (RN) in the State of Minnesota
  • 2 years' experience in a healthcare facility or home care field
  • Critical thinking skills, ability to follow standards/procedures, and make clinical judgments
  • Ability to work independently, adapt to change, and manage multiple assignments
  • Excellent written and oral communication skills, interpersonal communication, and teaching skills
