Job Description
The RN Case Manager provides direct care and oversees home health services for clients in the community, developing and implementing goal-centered care plans and working with the client, family, and healthcare team.

Requirements

  • Current license as a Registered Nurse (RN) in the State of Minnesota
  • Proof of negative TB testing
  • Access to a dependable automobile and valid driver's license
  • U.S. Citizenship or valid Alien Work Permit
  • Background studies and screenings
  • 2+ years' experience in a healthcare facility or home care field
