Job Description
The RN Case Manager provides direct care and oversees home health services for clients in the community. The position involves traveling to clients' homes, managing schedules, and working with healthcare teams to develop care plans.

Requirements

  • Currently licensed as a Registered Nurse (RN) in the State of Minnesota
  • Proof of negative TB testing
  • Access to a dependable automobile
  • Valid driver's license and proof of current car insurance
  • U.S. Citizenship or valid Alien Work Permit
  • Pass initial and ongoing background studies and screenings
