Home Health RN Case Manager provides direct care and oversees home health services for clients in the community, developing and implementing goal-centered care plans. Requires 2+ years of healthcare experience and MN RN license.

Requirements

  • Currently licensed as a Registered Nurse (RN) in the State of Minnesota
  • Proof of negative TB testing
  • Access to a dependable automobile and valid driver’s license
  • U.S. Citizenship or evidence of valid Alien Work Permit
  • Disclosure of any conviction and criminal history records
