Job Description
The RN Case Manager provides direct care and oversees home health services for clients in the community. Collaborates with the client, family, and healthcare team to develop and implement goal-centered care plans. Provides skilled nursing care to clients and clinical oversight to professional and unlicensed personnel.

Requirements

  • Currently licensed as a Registered Nurse (RN) in the State of Minnesota
  • Proof of negative TB testing
  • Access to a dependable automobile
  • Valid driver's license and proof of current car insurance
  • U.S. Citizenship or evidence of valid Alien Work Permit
  • Disclosure of any conviction and criminal history records pertaining to any crime related to the provision of health services
