Role OverviewThe Case Manager for Home Health provides nursing care in accordance with the client's plan of care, and includes comprehensive health and psycho-social evaluation, monitoring of client's condition, health promotion and prevention, coordination of services, teaching and training activities and provision of direct nursing care and other duties as deemed necessary.
What You Will Do
Provides direct nursing care, conducts initial and ongoing comprehensive assessments of home health patient needs, evaluates outcomes of patient care, and identifies discharge planning needs.

Requirements
- Graduate of accredited nursing program
- 2 years of RN experience with one (1) year experience with adult IV expertise required
- Licenses and Certifications: (RN) REGISTERED NURSE by Board of Nurse Examiners for the State of Texas (or BNE Compact State)
- Valid Texas driver's license, proof of driver's insurance
- BASIC LIFE SUPPORT accredited course by the American Heart Association (AHA) or American Red Cross (ARC)
