Job Description
We are seeking a Home Health RN Case Manager to provide direct care and oversee home health services for clients in the community. The RN Case Manager will develop and implement goal-centered care plans, provide skilled nursing care, and build relationships with clients and their families.

Requirements

  • Currently licensed as a Registered Nurse (RN) in the State of Minnesota
  • At least two (2) years' experience in a health care facility or home care field
  • Ability to work independently with minimal supervision and adapt to change
  • Excellent written and oral communication skills
  • Basic computer skills and willingness to learn new software programs
