Job Description

As a Home Health RN Case Manager at Adara Home Health, you will provide direct care and oversee home health services for clients in the community. You will work collaboratively with clients, their families, and the healthcare team to develop and implement goal-centered care plans.

Requirements

Currently licensed as a Registered Nurse (RN) in the State of Minnesota
Proof of negative TB testing
Access to a dependable automobile
Valid driver's license and proof of current car insurance
U.S. Citizenship or evidence of valid Alien Work Permit
Background studies and screenings
