Role OverviewThe Home Health RN Case Manager is responsible for planning, organizing, and directing Lifespark services while ensuring compliance with regulatory requirements and best practices. The professional nurse builds from the resources of the community & care team to plan and direct services to meet the needs of individuals and families within their homes and communities.
What You Will Do
The RN Case Manager completes an initial assessment of client and family to determine home care needs, provides a complete physical assessment and history of current and previous illness(es), and develops a care plan that establishes goals based on nursing diagnosis.
Why It Might Be a Fit
The ideal candidate will have 2 years' RN experience in healthcare, preferably in home care or hospice, and a basic understanding of OASIS. Experience in customer service, supervising caregivers, and knowledge of Medicare Home Health & Hospice regulations are also preferred.
Requirements
- 2 years' RN experience in healthcare
- Medicare Skilled Nursing experience and basic understanding of OASIS
- Experience in customer service/direct member contact field
- Experience supervising caregivers
- Knowledge of Medicare Home Health & Hospice regulations
- Graduate of an accredited school of nursing
- Current Minnesota Nursing License
