Role OverviewDay City Hospice is a local, patient-focused hospice organization seeking a compassionate RN to conduct pre-admission evaluations and facilitate admissions for patients and families. This role involves collaborating with an interdisciplinary team to provide exceptional end-of-life care.
What You Will Do
Conduct pre-admission evaluations to determine hospice eligibility, educate patients and families about hospice care, facilitate the admission process and obtain informed consent, perform comprehensive nursing assessments, develop initial plans of care, and deliver interventions promoting comfort and dignity.

Requirements
- Graduate of an accredited Registered Nursing program
- Current RN license in the state of Ohio
- Current CPR certification
- Hospice experience preferred (1 year preferred)
- Strong clinical judgment and ability to make independent decisions
- Excellent communication and interpersonal skills
- Ability to work compassionately and professionally with patients, families, caregivers, and healthcare professionals
- Knowledge of quality assurance and quality improvement practices
