What You Will Do
Conduct comprehensive adult/geriatric assessments, evaluate and manage patient care plans, and maintain continuity of care in a hospice setting.

Requirements
- Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice
- Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care
