What You Will Do
Key responsibilities include taking and processing Palliative Care referrals, providing support to the Palliative Care Nurse Practitioner and Medical Director, scheduling visits to support goals of care, coordinating and communicating patient care goals, and ensuring accurate documentation.

Requirements
- Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN) licensure in the state of practice
- A minimum of three (3) years of nursing experience
- Palliative care, hospice, or home health care experience is preferred
- CHPN (Certified Hospice and Palliative Nurse) is preferred
