Job Description
Join Care Initiatives Hospice as an RN Case Manager to provide comprehensive care that truly matters to patients and their families during their final stages. Our RN Case Managers go above and beyond to meet patients and families where they feel most comfortable and address their unique needs.

Requirements

  • Active Iowa RN license
  • Active CPR certification
  • Hospice or palliative care experience
  • Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred
  • Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region
  • Home access to the internet
  • Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment
