Role OverviewEnhabit Home Health & Hospice is seeking a Hospice RN to provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.

Perform services in accordance with the physician's orders and the established plan of care (POC), ensuring delivery of care is compliant with federal and state regulations.

Requirements
- Must be a graduate of an approved school of nursing education.
- Must be licensed in the state where they currently practice.
- Must have at least one year experience as a licensed registered nurse (RN).
- Must have basic demonstrated technology skills, including operation of a mobile device.
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
- Must possess CPR certification for the healthcare provider
